A fantastic opportunity has arisen for a Social Worker to join a newly established Fairer Funding Team.
As a Social Worker you will be an integral part of the Local Authorities new Fairer Funding Team.
Your extensive expertise and knowledge of Continuing Health Care (CHC) and Children’s Continuing Care will assist in ensuring individuals are supported to achieve their personalised outcomes utilising a strengths-based practice framework whilst also appropriately accessing alternative funding streams such as CHC.
Responsibilities will routinely include Social Care assessments for adults, carers and children, along with completion of reviews and any subsequent care and support planning and any related adult safeguarding enquiries.
This will require accurately maintaining client records and ensuring that these records are logged correctly on to the departments IT system along with strong report writing and communication skills.
You will also be supported with regular supervision and performance management sessions.
Along with a relevant qualification you should have relevant knowledge and experience in Adult Social Care either in a formal or informal capacity and be able to fully meet the requirements of the post and cover the whole geographic area of the City either by possession of a current and valid driving licence or by access to public or other transport.
