This is a fixed term post for 1 year.

We are looking for a fixed Term Shared Lives Coordinator within our Shared Lives Stoke Service.

 

The Shared Lives Stoke Service matches people who need care and support with an approved carer.

 

The carer shares their family and community life and gives care and support to the person with care needs.

 

The Coordinators role involves:

 

• To recruit, assess, support and deliver training of new carers, providing the specialist knowledge they need to offer the right support to those placed in their family

 

• Matching of people into shared lives placements, monitoring and supporting these placements

 

• Develop and support effective promotional and recruitment campaigns

 

• To contribute towards general service development shaping our services for the future

 

• Participating in the ‘Out of Hours’ support system for carers and to be responsive to crisis and emergencies.

 

• To support and supervise carers, ensuring that they are engaged, enabled, empowered, valued and motivated to provide the best possible care to the people they support

 

• Organise and attend support groups for approved carers as required.
