The Community Development Team within Adult Social Care and All Age Commissioning are looking to recruit a Locality Connector who will provide advice, guidance and support within our Community Lounges.
Job Purpose:
- To ensure that people are empowered to lead an independent life with community support.
- To ensure that the person is listened to and a strength-based conversation is utilised to assess and support people who attend a Community Lounge.
- To lead and coordinate a series of Community Lounges across the Locality, working across the system with partners to identify challenges local people are facing and have an improved sense of well-being through the provision of outcome focused support.
